Patient Information
To begin the treatment process for new patients, they will need to fill out a “New Patient Information Form.” This form will cover the patient’s medical history, current condition, allergies, insurance and any other pertinent factors we need to know. After gathering this information, we will be able to determine the most effective treatment for the patient’s needs and customize it.
